Support to understand your NDIS plan

Support Coordination is a capacity-building service, funded by the NDIS that helps you work out how you would like to implement your NDIS plan.

Our experienced team works with you to help you make the most of your NDIS plan. We will work with you to build your capacity, connect with the right supports and guide you through every step of your NDIS journey.

We proudly support people in communities across Echuca, Moama, Shepparton, Yarrawonga, Bendigo, Swan Hill, Deniliquin—and everywhere in between. Whether you prefer in-person, phone or online support, we’re here to meet you where you are.

We can assist you to:

Explore and choose the right providers and services
Negotiate service agreements and pricing
Organise assessments and manage budgets
Liaise with plan managers and link to housing, education, transport and health services
Build your confidence and skills to enable you to manage your plan independently

We offer expert help with:

Scheduled Plan Reviews: Assess progress, value for money and future goals.
Unscheduled Plan Reviews: Support you through changes in circumstances.
Review of Reviewable Decisions: Help you challenge decisions that don’t meet your needs.

Whether you’re moving out for the first time or relocating, we help you explore housing options and plan life transitions with confidence.
